Wash the grapes, removing the stem
For the cream, place the condensed milk, milk, yolks and butter in a pan on the fire, cook until the cream thickens and let it cool.
Chop the chocolate into cubes, to melt in a bain-marie or microwave with the cream
Assemble a layer of grapes, then the cream and over the chocolate. Decorate with grapes if you prefer.
